Gift Cents - Enrollment

Becoming a GIFT CENTS SCRIP customer is simple! Please review the list of Enrollment Requirements and
then complete the forms provided below.


	You must be a non-profit organization. You must have proof of your non-profit status - Tax Exempt ID number or Certificate of Exemption or Letter of Determination Banking information if you will be using our Electronic Bank Draft (EBD) direct payment plan. Include a completed EBD Form and a copy of a voided organization check. Enrollment form filled out completely. Be sure to indicate whether the shipping address you provide belongs to your organization, your residence, or a business.


	Print out and complete our enrollment forms: • Enrollment Form • Direct Payment Plan/Electronic Bank Draft (EBD) Enclose a copy of your Letter of Determination or evidence of tax exempt status.
