Gift Cents - Enrollment

Becoming a GIFT CENTS SCRIP customer is simple! Please review the list of Enrollment Requirements and
then complete the forms provided below.


	You must be a non-profit organization. You must have proof of your non-profit status - Tax Exempt ID number or Certificate of Exemption or Letter of Determination Banking information if you will be using our ACH Debit Program (include a completed ACH form and copy of a voided organization check ). Enrollment form filled out completely. Be sure to specify if your shipping address is to your organization, your residence, or a different business.
